  Director of a group of high quality professionals within our Case Management Department. Our area encompasses a hybrid dyad model which consists of utilization review, discharge planning, social services support and clinical documentation improvement. We also cover transitional care management, care management and chronic care management for the outpatient setting. We have a very strong focus on value based care, quality management and revenue cycle management as an extension of our utilization review/denial management function.
