  Responsible for supporting a case load of Alcoholic and Drug rehab management plans in Homeless hostel in Dublin. I worked with both STA and LTA clients, and delivered coordinated, holistic support, through internal and external specialized teams. As a Project Worker, I take on the responsibility of ensuring the efficient and effective running of services on a daily basis. We work closely with the DRHE, Dublin City Council in relation to delivering service provision through risk assessments, support planning and holistic needs assessments. Using OTIS and Pass Systems to coordinate and update information such as wellbeing checks, S&D Reports, Incident/accident reports, Specific Resident case notes, Shift Handovers and other duties.
