  Older Adult Mental Health provides a continuum of care to seniors throughout Contra Costa County with teams that provide outreach, engagement, and on-going services to the most imperiled seniors in the county.Member of the Intensive Care Management Program providing field-based services to older adults (60+) diagnosed with a chronic and severe mental illness in their homes, board and care homes, shelters, etc. Work with a multi-disciplinary team, which includes a Geropsychiatrist, Nurse, Mental Health Clinicians, and a Community Support Worker. Provide case management services to older adults, collaborate with family members and other service providers, and provide crisis intervention/risk assessment as needed. Clinician on the IMPACT Program providing short-term Cognitive Behavioral and Problem-Solving therapy to older adults (55+), who often have co-occurring medical diagnoses, such as diabetes, cardio-vascular disease, and/ or chronic pain as well as moderate to severe depression, anxiety or Post-Traumatic Stress Disorder.
