  Reviewed and classified over 1300 unidentified medications. This information was used by researchers in the LIFE Study. The Lifestyle Interventions and Independence for Elders Study is a phase 3 randomized controlled trial designed to preserve the ability to walk in older adults.

  I am a pharmacist working as an ambulatory care clinical coordinator for Oregon's safety net population (OHP, Medicaid). I have responsibilities in program development and patient care. I work patients who have serious, life-limiting illness. We use an interprofessional team of physicians, pharmacists, nurses and social workers, to ensure the best continuum of care for patients with advanced illness and palliative care needs.
