      Program Manager, Population Approach to Health (PATH)

      • Implemented four PATH programs: Diabetes, Depression, Hypertension and Diabetic Retinopathy Screening. Led each PATH team of 1-2 physicians and/or ancillary clinical staff.• PATH Depression and Diabetes met target quality goals for value-based payor contracts. Pilot cohort results showed a decrease in elevated disease states, a decrease in missing disease metrics for individual patients, and an increase in primary care engagement. PATH Depression reached a 58.8% remission/response rate (target 40%); PATH Diabetes reached 46% A1C control rate (target 35%).• Developed and implemented supplementary depression pilot program testing new panel management workflows for clinic social workers and health navigators, under budget. Compared to control clinics, pilot clinics saw 16.6% greater depression measurement using standard tool (PHQ9), 8.8% more patients in response/remission (PHQ9<9 and/or 50% improvement), and 4.9% more PCP visits, meeting or exceeding pilot goals. • Launched first Epic “Clinical Pathways” at UW Ambulatory Clinics. >3000 patients added to the Depression Pathway from June-December 2018. The Hypertension Pathway Pilot showed a 22.2% increase in correct blood pressure re-measurement and a 10.2% increase in HTN control. Show less
