  •The Case Manager meets weekly with each client to check on progress, discuss any concerns, and create action plans.• The Case Manager completes an initial assessment to determine the client's needs, areas of deficit, and creates a treatment plan with the client.• Throughout treatment, the case manager is teaching the clients life skills such as financials, legal issues, paying bills, resume building, interview skills, and much more. These skills relate to their treatment plan goals and the Case Manager is pushing the client to be able to complete these skills independently.• The Case Manager uses the eight dimensions of wellness and other therapeutic modalities to help the clients achieve their vocational goals and obtain all of the resources they need to transition to a lower level of care.• The Case Manager obtains supervision and consultation from other staff at the facility including the Program Director, therapists, and medical team.• The Case Manager communicates and coordinates care with outside providers and family/friends.• The Case Manager works on a detailed relapse prevention plan with each client.•The Case Manager coordinates discharge planning which includes aftercare appointments, sober fun activities, meeting schedules, housing, social support, spirituality, and more.• The Case Manager documents all activities done with the clients and updates their treatment plan frequently.• The Case Manager works to help each client resolve social issues, develop healthier habits, and create overall balance in their life.
