  During my 400 hours placement, I facilitated and developed daily School Leaver Employment Supports (SLES) group programs for young people with disabilities to support them in achieving independence, find and maintain employment. I developed group programs that would range from self-care, personal hygiene, relationship-building, cyber-security, money budgeting, resume-building, interview-skills training and work-experience training. I developed a strong rapport with many of the clients which allowed me to conducted vocational profiling assessments and supported to overcome their anxiety of applying for work experience.   During my final placement, I supported my supervisors in facilitating the planning, development and implementation of person-centered NDIS support plans that would assist clients with disability to achieve their NDIS goals. Due to the complexity of some client’s disability, I have work in an interdisciplinary team of allied health professionals to ensure that all aspects of a client’s lifestyle were addressed and supported. I have also work alongside clients towards assisting them towards attaining social housing by gathering supporting documents from therapist and completing several forms with the client’s consent and active involvement.
