      Responsible for the oversight of the Community Care for the Elderly and Home Care for the Elderly programs funded by the Department of Elder Affairs and administered by the Alliance for Aging the state of Florida’s designated Area Agency on Aging for Miami-Dade and Monroe county. The contracts provide supportive services for the Elderly facilitating their ability to stay home with the support they need to maintain their independence for as long as possible preventIng pre-mature institutionalization. These contracts are critical to the growing aging population in various ways. The Community Care for the Elderly program provides a wide variety of services that are identified with the completion of the Department of Elder Affairs comprehensive assessment. The assessment determines client needs and care plans are developed accordingly. It approaches each individual using a person-centered approach ensuring available funding for services are maximized. Programmatically, the client is required to make co-payment which in turn contributes to the funding availability allowing flexibility in revising active participants care plans if significant changes have occurred and allows for more clients in need to be served. The Home Care for the Elderly program focuses more on the support of the client’s caregiver; an individual residing with the client who provides daily care for the client. This program simply requires that the identified caregiver actively resides in the same home and is willing and able to continue caring for the client. The main support service rendered here is a subsidy payment. The amount is determined by the client’s income.Contract monitoring to include various programmatic requirements, quality assurance, and outcome measures that validate contractual requirements are being met.       LTC Plus Case Management

      Responsible for the case management of individuals 18 and older who have met the financial and medical eligibility for the the Statewide Medicaid Managed Care Longterm Care Program (SMMC LTCP). Eligible recipients undergo a comprehensive assessment that determines their needs and the development of a Care Plan that targets gaps by implementing services that support on-going independence preventing premature institutionalization, which ultimately reduces costs, expands coverage, and improves the individuals quality of life and health. Additionally, for those who are a part of SMMCLTCP also known as the Home-Community based Waiver program, the biggest benefit they get is the ability to stay in their own home or community, instead of being sent to a care institution. Staying at home has been shown to have many quality of life benefits, especially for seniors who get to age in place. Home is comfortable and familiar, allowing those to feel at ease compared to the jarring change of environment that a nursing home or other residential facility can bring. Not only has this been shown to improve recovery time after an injury or surgery, but it’s also been shown to improve mental health and reduce complications from dementia as well as other cognitive and/or mental health diagnoses. It costs about 50% less of taxpayers dollars to provide supportive services in the home. This means that without these cost effective supports an elderly for instance suffers a fall while they shower, leads to a hospitalization, then to a rehabilitation and more often than not end up institutionalized costing anywhere between $30,000.00 and up a year per individual. In contrast, an average Care Plan for a client at home cost about $12-15K a year and that is for a “fluffed up” Care Plan. Show less
