      Monash University organised a 2-week simulated placement during the COVID pandemic as a safety alternative. It provided me with an opportunity to familiarise and practice myself with the roles of an occupational therapist within a telehealth setting. All learning and meetings were conducted via Zoom. This placement consisted of two settings including community rehabilitation and mental health.In the community rehabilitation setting, I successfully administered an initial assessment and a home assessment through Zoom meetings. I had a chance to practice my communication skills, clinical reasoning and observation skills to gather information through semi-structured interviews. Thus, I was able to identify the environmental facilitators and barriers, as well as potential occupational performance issues encountered by my clients.In regards to the mental health setting, I administered a Canadian Occupational Performance Measurement (COPM) assessment to identify client’s needs, goals, and possible interventions. The COPM allows me to practice setting client-centred goals with my clients collaboratively. In both settings, I had a great chance to practice my communication skills, including questioning, active listening and providing reassurance for my clients, which helps to build rapport with them. Besides, I realised the importance of involving client’s significant others in the therapeutic process, such as family, caregivers, and health professionals. Furthermore, I practiced note-taking during the interviews, writing SOAP notes and progress notes, and completing referral forms. Show less

      I completed a 6-week geriatric placement at Monash Health, Kingston Centre, which took place in an in-person hospital setting. During this placement, I had the opportunity to conduct rehabilitation sessions, make home visits, and administer assessments.Ward South 6 is a sub-acute rehabilitation unit aimed at providing further support and improving the functioning and independence of clients. The clients over the age were over 65 and did not require ongoing medical care but were not functionally ready to move on to the next stage of care. They presented with a variety of diagnoses, such as dementia, delirium, stroke, and Parkinson’s disease.Common occupational performance issues among my clients included difficulties in engaging in self-care activities, including toileting, light meal preparation, dressing and showering.My role as an occupational therapist within this setting:1) Gathering information through administering assessments2) Identifying and setting SMART goals with clients collaboratively4) Writing documentation and providing handover of client’s progress to the multi-disciplinary team5) Organising and implementing joint session with other health professionals6) Discussing and identifying appropriate discharge destinations with clients and their significant others7) Prescribe equipment8) Case presentations within the occupational therapist teamAssessments conducted within this setting:1) Initial assessment2) Functional assessments related to self-care activities3) Home assessment4) The Orientation Log (O-Log assessment)Interventions delivered1) Self-care retraining2) Cognitive retraining3) Discharge planning4) Equipment prescription5) Delirium ManagementI was able to articulate my clinical reasoning to identify appropriate interventions and prescribe equipment for my clients with my supervisor that fit the gap between their previous and current occupational performance, such as long-handled aids, shower chairs, and toilet frames Show less

      In the third and fourth years of my university studies, I served as a project manager of a participatory community project.       I completed a 10-week placement at Each, Childhood Intervention Services which is a community pediatric setting. I worked with children aged 3-8 presenting with a variety of conditions impacting their development, such as developmental delays, autism spectrum disorder, sensory processing issues, intellectual disability, and trauma history and their family. My role was to support children's development and learning in areas including fine motor, emotional regulation, handwriting, and social skills and also their family to be involved and support the growth of the children.Common occupational performance issues among my clients included difficulties in engaging in self-care activities, learning, and leisure activities, such as dressing, handwriting, and making friends.My role:1) Session planning for children and their family2) Home and school visits weekly or fortnightly to observe children’s behavior in these environments 3) Organising play-based activities for children to support their fine motor, functional grasping, and emotional regulation4) Administering assessments to assess the current occupational performance of the children5) Documenting session progress and formulating recommendations to parents6) Engaging and supporting children's significant others to be involved to support children's growth and development7) Completion of a non-clinical project to explore external services for children and familiesAssessments conducted within this setting:1) Miller Function and Participation Scales2) Sensory Profile 23) Observational assessmentPlay-based interventions delivered include playdough, pop the pig, pop the pirate, threading activities, toileting books and role play, emotion cards, peg puzzles, container play, paper drawing with crayons, nesting dolls, and frog hopper game.
