"Home Health Care Quickly Hones Critical Thinking, Judgment, Organization, Prioritizing, and Interpersonal Effectiveness Skills and Creates Self-Reliance Competence"Transitioning the patient from the hospital or any other continuum of care organization to home placed the patient, their family, and clinical home caregiver in a circumstance that is almost wholly-dependent upon the assessment, listening, observation, documentation, and communication skills of the RN.• I managed patient transitions of care through the hospital and then to the appropriate post-acute setting attaining third-party payor approval and proper documentation for any services provided to support the patient's status, level of care, and DRG for complex disease management utilizing nursing assessment, critical thinking, judgment, and clinical skills.• As a Managed Care Nurse, I was boots on the ground liaison between patients, doctors, healthcare providers and insurance companies. • Although it was not planned, I acquired specialization as Maternal/Child and Pediatric Case Manager for patients in challenged inner-city, urban setting.• I managed antepartum/post-partum women, well-baby, premature infants, pediatrics, pediatric oncology, and hematology; and adults with acute, chronic, and multisystem disease processes. Delivering independent in-home care honed clinical problem-solving and decision-making skills.• My keen observation skills was not limited to seeking signs of failing skin integrity. I evaluated patients for signs of abuse during exams and visits, and reported as indicated. • Facilitated care between ancillary services and the health provider was essential to implemented plans of care, tracking pediatric growth and milestones, and wellness promotion/disease prevention education. Show less "When Child Incontinence Exists in a Family, the Family Becomes a Quasi-Patient Too"Seeking to understand what is happening or rather not happening is crucial to managing incontinence. Behavioral and/or physiology components can be at the core of voiding symptoms. Uncovering children with oppositional defiant disorder, anxiety, depression or autism (among other conditions) is fundamental to sourcing the causes of urinary incontinence issues. If you are the parent or caregiver of a child of any age with incontinence, get your child evaluated by a pediatric specialist. A child will not grow out of bed-wetting or incontinence voiding if it is a physiological issue.• I oversaw urology clinical protocols for comprehensive diagnostic and therapeutic services for an extensive range of children urological disorders resulting in incontinence at all ages. • Operated a urodynamics laboratory and comprehensive diagnostic radiographic and radionuclide evaluation to assess bladder performance; performed in the OR. • Interviewed patients and families for complete health history, diagnosed and managed acute and chronic conditions; performed history and physicals on preoperative patients. • Ordered, interpreted, evaluated, and integrated findings, records, referrals, and diagnostic tests results. Performed microscopy and bladder ultrasounds; interpreted the results. • Consulted with Physician as needed following assessment; wrote referral letters and reports. • Initiated treatment plans, prescriptions, patient/family education, and urological follow-ups.Life gets better for children when bladder issues are addressed. Anticipate improved self-esteem, socialization and independence following incontinence treatment. Show less
