As the Health Systems Specialist--Risk Manager, I work within the Quality Resource Department to systematically identify, evaluate, reduce and/or eliminate risk at the Medical Center. I also monitor the occurrence of adverse events and situations arising from operational activities and environmental conditions at the Medical Center. The overarching goal of the program is to improve patient care. I coordinate and manage programs such as Peer Review, Administrative Investigative Boards, Fact Finding Investigations and Tort Claims. I work closely with the Patient Safety Manager, assisting with Root Cause Analysis activities and patient incident reporting. I also assist other members of the Quality Resource Team with different functions at VA. I support my department with Office of the General Inspector (OIG) , Commission on Accreditation of Rehabilitation Facilities (CARF) as well as The Joint Commission during survey visits.February 2018 Co-Chair of Employee Threat Assessment Team (ETAT). Show less As the SUD Specialist I supported the HUD-VASH Program by providing continuity of care between Homeless Programs through services to Veterans with SUD issues as they transition from programs such as Domiciliary or Residential Rehabilitation and Treatment Programs (RRTPs), Health Care for Homeless Veterans Contracted Housing, and Grant Per Diem Programs to HUD-VASH. I provided on-going substance use disorder case management and early treatment interventions to promote sustained stability, abstinence or harm reduction for Veterans who were enrolled in the HUD-VASH subsidized permanent housing program. The Veterans I worked with were frequently homeless due to their SUD activities, so providing SUD treatment support and early intervention promoted recovery, improved quality of life and successful permanent housing. I provided direct, concurrent clinical care using established evidence-based practices and Motivational Interviewing approaches for Veterans with co-occurring conditions of homelessness and SUD. I conducted treatment groups for Veterans with significant SUD concerns. I coordinated with existing SUD specialty services to ensure that the HUD-VASH Veteran participant receives continuing care for SUD, as appropriate and as needed, and after permanent housing has been found. I was a member of a specialized team whose goal is to improve Veterans quality of life through the provision of evidence-based outpatient treatments for substance-related and addictive disorder. Treatment includes employing a directive, non-confrontational approach focusing on current stressors, affect management, cognitive distortions, and behavioral change. I conducted assessments and developed treatment plans that identified goals of treatment based on these assessments and in collaboration with the Veteran and family, where possible, and with the interdisciplinary treatment team.
