Directed multiple aspects of the 2022 Community Health Needs Assessment (CHNA) for all 7 Bon Secours Richmond acute hospitals, and now lead the resulting Community Health Improvement Plan (CHIP) for 2023-2025 for all of Bon Secours Richmond.• Accomplishments:− Led or co-led multiple aspects of the 2022 CHNA for the Bon Secours Richmond service area, including data analysis of 1500 survey responses, a review of publicly accessible data regarding health issues and root causes, coordinating town hall community conversations, and leading key informant interviews with nonprofit, healthcare, and government leaders across Virginia− Directed launch of Unite VA social needs referral tool across Bon Secours Richmond to outpatient, inpatient & point of entry clinical teams− Managed relationships with a diverse array of community partners and nonprofits− Represented Bon Secours Richmond in the 2022 Chesterfield Community Health Assessment process.− Served as voting member of the Bon Secours Richmond Community Benefit Investment Grantmaking and Sponsorships Team− Implemented an ongoing Community Engagement tracking process for the entire Community Health team− Led data analysis of Bon Secours targeted service areas, including creation of one-page snapshot reports incorporating publicly accessible data and CHNA survey data to create a picture of the communities served by Bon Secours Show less • Managed a clinical direct service staff of seven RN, LCCE, and IBCLC educators that lead interactive community education to expectant families.
