  I served as a Masters level Palliative Care intern for two semesters at UPMC Presbyterian and Montefiore Hospital. I worked within a multi disciplinary team of psychologists, nurses and doctors. My role was to serve as a liaison between the multidisciplinary team and the families and patients we serviced. I coordinated discharge planning, completed psychosocial assessments, educated families, and also offered support to patients and families regarding end of life issues.

  My role as a Medical Case Manager is to develop a comprehensive individualized service plan for those living with HIV/AIDS. Key activities include (1) initial assessment of service needs, (2) coordination of services required to implement the plan, (3) client monitoring to assess the efficacy of the plan, and (4) periodic reassess the efficacy of the plan at least every 6 months, as necessary over the life of the client. It includes client-specific advocacy and/or review of utilization of services. This includes all types of case management including face-to face, phone contact, and any other forms of communication.Our goals are to provide early access and maintenance of treatment, medical care, and social services; to delay HIV progression and transmission of HIV; and to promote and foster self sufficiency and independence through a client centered, strength based approach.
