Serve as the key point of contact for all Avamere Skilled Nursing facility Social Service Directors, Assistants and Discharge Planners; to facilitate discharge into Avamere Community Based Care Communities that are not admitting back to home or to a skilled property. Key responsibilities include: 1. Reaching out to family members and/or residents to discuss transfer to Avamere assisted living and facilitation of a smooth transition. 2. Ensure assessment will be scheduled in a timely fashion. 3. Discuss care needs and facilitate transfer or medical records from the skilled property to the assisted living. 4. Arrange transportation from the skilled to the assisted living on admission day to the assisted living.5. Keep and log all referrals in an Excel spreadsheet. 6. Participate in ongoing meetings with discharge planners from Avamere skilled properties as well as admission directors from the assisted living properties to strengthen communication and sure referrals. 7. Assist DNS with evaluation if the building is unable to complete assessment with in 48 hours.
