      The goal of this project, under the Department for International Development, was to improve the health and nutritional status of 39,975 children under the age of five and 42,565 women in Karamoja, Uganda. This was achieved by utilizing the Care Group Model through training 25 health promoters and 2,027 mothers so that they would be equipped with the appropriate knowledge to educate others on better health practices. In addition, 16,000 men, 600 community health workers, and 300 community/religious leaders were trained to promote behaviors that will improve maternal and child health in varying socioeconomic groups in the region.My duties included conducting barrier analyses on the target population of a new Maternal Child Health project and assisting with designing the new MCH curriculum. I studied survey data to complete log frames and reported program targets reached to measure output, outcome, and impact indicators affecting progress and cost. I also provided outreach and development aid through health education and community engagement by networking with officials. Additional duties were as follows:• Provided health education and training through discussion groups & community engagement• Edited final reports & evaluate impact indicators for grantors; assess base line and end line data • Participated in collaborative meetings with NGOs, health supervisors, and government officials• Trained health promoters on conducting a barrier analysis using interview questionnaire data • Supervised health promoters and collaborated on best practices for health program development• Completed administrative tasks (logistics, expense reports, log frame analyses, and scheduling)• Represented Samaritan’s Purse at the national MCH meeting at Uganda’s Ministry of Health Show less

      In September 2017, I transitioned within Prisma (formerly Palmetto) Health from working on PCMH under Population Health Management to coordinating the Diabetes Prevention Program (DPP), a nationally recognized, CDC-approved program that encourages a 5-7% weight loss in pre-diabetics to decrease incidence of Type II Diabetes. My tasks include supervising nine health coaches who teach yearlong cohorts, coordinating the logistics of the program, networking with clinicians to increase referrals into the DPP, orienting new participants, and supervising assessments, support groups, and graduations, among other duties. Our DPP was one of three national DPPs that were selected to partner with the American Medical Association on a five-month collaboration aimed at increasing physician referrals into national DPPs. In addition, our DPP is one of only two national DPPs that have been selected by the CDC to undergo a rigorous two-year evaluation as a nationally high performing DPP. I was interviewed on our local news station, WLTX 19, to discuss symptoms, risks, and prevention of Diabetes for Diabetes Prevention month in November. Show less As a Program Coordinator in Population Health Management, I worked on the Patient Centered Medical Home (PCMH) program under NCQA, which is "a model of care that aims to transform the delivery of comprehensive primary care to children, adolescents, and adults." My role involved evaluation, education, and assistance to staff at physician offices within the Palmetto Health/USC Medical Group network to fulfill specific requirements and CMS guidelines that ensure the highest quality of care to patients. I evaluated and assessed gaps in care, opportunities for improvement, and efforts to maximize the involvement and engagement of staff. I ensured that best practices were implemented into workflows, leading to improved care, utilization of resources, and decreased cost. The PHF curriculum is for young women seeking mentorship and biblical advice and can be used individually, with a mentor, or in a small group.
