  Activities Director and Social Services Assistant

  I began as a summertime Volunteer working in reception and co-facilitating activities (I was a minor at the time). After I turned 18, I was interviewed and offered the position of Activities Director due to my experience and volunteerism. From there I managed the life enrichment portion of all patient care plans, for a skilled nursing rehabilitation of 50+ beds. I also expanded my role to provide daily support in social services, assisting with the coordination of admission and discharge including admission information gathering, and coordinating discharges to private residences or other healthcare settings. I also provided periodic tours resulting in admissions and aided the medical records department/ resident care managers in support of survey readiness and chart auditing once monthly.   Overseeing the daily operations of a six-bed adult family home focused primarily on dementia and memory care. In addition to overseeing a 24-hour care home, I provided compliance and survey readiness for the other five homes within the complex.
