Developed an integrated network of community services, including primary care, adult day care and home care.•Instituted financial management controls to operate financially challenged CNR Health Care Network at a surplus.•Transformed the cultural focus of CNR from a Jewish-sponsored nursing home in Crown Heights/Bedford Stuyvesant to an organization focused on caring for the area's predominantly black community. Developed a system to reach out to community board, organizations, and churches with active involvement resulting in strong community support for CNR.•Recruited and developed a racially and culturally diverse leadership team that reflected the community's population.•Contributed to redevelopment of CNR's neighborhood by working with community board and local politicians to construct building on vacant city land to house new offsite adult day care and home care programs.•Opened offsite adult day care center in Queens, expanding continuum of long term care services from former Brooklyn-only focus.
