  Health Connect is actually three clinics in one. The Welcome Clinic provides primary care services patients who are new to JPS. The Post Hospital Discharge Clinic sees patients who don't have a PCP for their follow up care after admission or ED visit. In both of these clinics, we have navigators who help assist patients in transitioning to a Patient Centered Medical Home. The third clinic is the Extensivist program which sees patients who have been high utilizers of both ED and IP care. The Health Connect team helps patients learn to manager their care and to reduce ED/ IP encounters. Show less In this role I have worked with a team of Care Ambassadors to assist patients in navigating through the hospital network in order to obtain the services needed. We have been responsible for handling patient concerns and complaints and for resolving these issues in compliance with CMS and Joint Commission Standards. This requires excellent communication skills with patients and families as well as with other staff members including managers, directors and providers.Our team has been responsible for rolling out staff education for JPS Community Health. In the last several years, we prepared and presented PowerPoint and Video Presentations as various topics were rolled out though JPS, including Patient and Family Centered Care, Patient and Family Centered Medical Home and Customer Service topics.
