  As a Community Health Worker, I work with justice-impacted individuals to attain healthcare, vital documents, public assistance and overall reintegrate participants into society and their communities. I managed a caseload of 45 clients, an 80% increase from the initially assigned caseload. By helping my clients find primary care, mental health services and employment, I can overall increase their chances of successfully navigating reentry into society.Specific roles:• Tasked with 45 participants.• Enrolling the participant by conducting the bill of rights, consent, intake and needs assessment.• Building capacity within participants which can be teaching them how to better adapt to society.• Provide insights and expertise as a member of the Divisional Action Team for the Bureau of MentalHygiene’s Race to Justice Initiative to make our work more inclusive and equitable.
