      Renal social workers serve as a support system for patients and their family in adjusting to and understanding end stage renal disease (ESRD). Having ESRD is understandably challenging and overwhelming. Social worker is the advocate, and serves as a bridge in communicating patients’ needs to the treatment team members, which consist of the nephrologist, nurses, dietitian, social worker and, of course, the most important person, patient. Identify and remove barriers to patients by providing a broad range of social and family supports, connecting people with programs and services to help meet their basic needs, and work with individuals to solve relationship problems and enhance their overall social well being. The Mobile Response Team (MRT) operates AHS's mental health crisis triage for the Calgary Zone.MRT offers urgent mental health support, and / or consultation in crisis situations. They have the ability to arrange urgent psychiatry assessments and referrals as needed. MRT provides mobile and telephone mental health services, including: crisis intervention, crisis stabilization, trauma response, mental health education, professional and / or third-party consultations.

      Senior Project Coordinator

This project invites individuals who are interested in building their capacity as well-being, healthy relationships, and violence prevention peer-facilitators and advocates. Training, mentorship and support is provided for facilitators to lead well-being groups within their communities and evaluate the impact of this promising practice.
