The goal of the program is to help educate those in the community about diabetes and how to prevent and reduce the illness as well as complications.Provided Door to door outreach within the South Lawndale Community. To seek community members to participate in our intervention program for diabetics. In the field provided them with blood pressure measurements as well as Hemoglobin A1C readings if their predisposition to DM were high (based on a self assessed risk factors). Relay information about health resources available to those in the community if predisposition was indeed high. Regardless of Hemoglobin A1C readings, Educated participants and family about diabetes as well as aided in the engagement of better preventive methods. Show less
