Christian Health offers excellence in independent living, assisted living, skilled-nursing care, short-term rehabilitation (post-acute care), memory support, and mental health services. At Christian Health, I had the privilege to educate residents on the importance of completing an Advanced Directive form. Through the role of creating an Advanced Directive brochure and teaching one-on-one with each resident, I was able to provide information and tools to the residents. I was able to inform residents about the types of Advanced Directives NJ has and promote completing forms. - Identified the learning skills of each patient- Created an easy-read brochure - Residents became aware of different types of Advanced Directives in NJ- Less chance of residents not completing an Advance Directive.- Better understanding and knowledge from patients was recorded.
