      Podium Presenter Van Liere Convocation Feb 2013 Presented the results of a research project with Dr. Charles D. Ponte entitled "Role of the student pharmacist in medication reconciliation in a novel discharge follow-up clinic." The clinic was designed to decrease readmissions on the Family Medicine inpatient service at WVU Healthcare by seeing patients back in the outpatient clinic within 7 days following their discharge.
