  ADLIFE is an EU Horizon 2020 funded project, developing innovative digital health solutions to support the healthcare planning and care delivery for patients with multiple advanced long term conditions (we are focusing on people with Chronic Obstructive Pulmonary Disease (COPD) and/or Heart Failure (HF)). The team is made up of 11 different consortium members across Europe and Israel.The main aim of the ADLIFE project is to improve the quality of life of patients living with these conditions, through intelligent, digital, integrated and personalized care. Innovative digital solutions are focused on the early detection of care needs, to tailor dynamic and personalized care provision. ADLIFE will prove that intelligent, collaborative digital solutions can enable care teams, patients and caregivers to improve or better maintain health in patients with advanced chronic disease. The project will enhance patients' and caregivers' autonomy by encouraging them to participate in informed decision making by providing them with personalised care plans. Show less
