Ensured that the medical care teams understood the medical plan of care and were aware of the Soldiers non-medical goals. Identities unsheltered homeless neighbors through direct street outreach activities and through community referrals from mental health, hospitals, local law enforcement and municipalities and other social service agencies. Builds trusted relationships, provides advocacy, and assists with connecting individuals to benefits and housing. Attends team meetings, case conferences, training workshops, and community meetings as needed. Serves as the homeless management subject matter expert for the City of Lacey.• Coordinated the City of Lacey 2020 Point in Time Count; surveyed 65% more individuals than 2019.• Planned and executed the first Point in Time Count Family event; solicited over $2300 in private donations to purchase items to be donated at the event.• Requested by the Thurston County Health Department to be a part of the Hazardous Weather Task Force for the county – working to plan expansion of services and outreach to minimize illness and death among unsheltered homeless persons during normal and extreme weather.• Serves on the Vulnerability Index: Racial Equity Research Team to collaboratively work with advocates and other social service providers to focus on the Vulnerability Index tool for Single Adults and Veterans and ensuring equitable scoring of marginalized populations. • Serves on Safety Strategy Committee for Thurston County – working to develop services and proposals for the county aimed at supporting the safety of our BIPOC and trans participants who are targeted by racist and transphobic violence.• Partnered with the Athena Group to help them facilitate a community-driven strategic planning process on homelessness.• Serves a member of the Greater Outreach Workers League (GROWL) to coordinate outreach efforts into all areas of Thurston County.
