      Family Support Coordination (FSC) is a First Things First initiative that commits to provide families with the tools to set a solid foundation for all children. FSC is a referral line open to the public, free of charge, with the potential for short term case management for the families that live in the Central Phoenix zip code area and who have multiple and complex needs. This program is a partnership between seven agencies. Coordinators from these seven agencies collaborate to empower families and connect them to the services they need. Networking and doing outreach to promote this program has been an important part of this job to create a relationship with service providers and to raise awareness about the existence of this strategy.       The Screening Care Coordinator is the lead and responsible party to report to the program manager within the S.C.O.R.E (Screenings with Care for Outcomes and connections to Referrals and Education) team in the General Pediatrics Department at Phoenix Children's Hospital.
