The project objective was to improve patient safety and patient outcomes through efficient referral management and patient safety. - Involved with Clinical Redesign processes for Children's Health by strengthening health services through optimising nursing strategy and action plan-Monitored and reported on timelines, schedules, and risks to the Executives and stakeholders-Managed risks and issues identification, assessment, and mitigation activities -Managed business change management deliverables and resources required to ensure the successful implementation and adoption of the Referral Streamline Project and that it meets the business needs.
