      My first placement involved provision of occupational therapy services in collaboration with my placement supervisor with clients who have NDIS plans and live across the south eastern suburbs of Melbourne. Support was provided to individuals across the lifespan with a range of disabilities, including mental health, psychosocial, physical and neurological conditions.       My second placement involves provision of occupational therapy services in collaboration with my placement supervisor in the community outreach team. The service is funded by the Home and Community Care (HACC) program for clients under 65 years of age and the Commonwealth Home Support Programme (CHSP) for clients over 65 years of age. Support was provided to clients across Frankston and Mornington Peninsula whose physical health impacts their ability to live safely at home, complete their daily activities and access the community. The purpose of the service was to promote clients’ independence and safety in the home and community. I assisted in:• Gathering information on clients’ goals, mobility, transfers, cognition and home environment during interactions with clients and their families• Administering home assessments and making recommendations on assistive technology and home modifications • Exploring assistive technology options that meet clients’ needs• Trialling assistive technology with clients, such as long-handled aids• Developing care plans that included clients’ goals and steps to achieve these goals• Administering assessment and prescription of wheelchairs, motorised scooters and recliner chairs• Completing funding applications for assistive technology via Geat2GO and SWEP• Completing funding applications for home modifications via Mecwacare• Collaborating with a range of stakeholders, such as equipment suppliers Minder weergeven
