Accountable for referral volume and optimal payer mix through development and management of community partnerships with hospitals, nursing homes, assisted living facilities, mental health providers, Diversion Court, United Way, and Area Agency on Aging. Participated in budget planning with CFO and executive team. Interviewed and hired staff. Attended regular meetings for AHN ACO with coordinators, physicians, and other leadership. Consulted as mental health and dementia subject matter expert for business development team.* Strategized and oversaw growth of organization from 20 to more than 300 employees and expanded mental health program from 3 to more than 20 nurses and social workers. Provided partnership presentations to agencies and organizations and acted as voice of organization in community.* Brought home health mental health to older adults in rural settings, becoming preferred provider to facilities and organizations such as Area Agency on Aging.* Negotiated contractual partnership with physician group owned accountable care organization (ACO) with 8,000+ lives during genesis of health care system conversion to quality outcome reimbursement. Facilitated ongoing relationship and helped develop innovative service model that slashed rehospitalization rate to as low as 1.7%.
