      I was able to provide community treatment to individuals with a mental health disability. I worked in conjunction with others to develop and update client plans, meet with individuals to progress towards achieving goals outlined within these plans (e.g. vocational development, attending weekly groups, grocery shopping, attending appointments, apartment hygiene, personal hygiene, etc.).       In this position I worked together with an RN case manager, to provide medicaid/long-term care case management services to members of CCCW's Family Care program. When providing case management, timely, objective, person centered planning was essential. Education, and empowerment based on research was provided to assist members with decision making that our team felt would assist the person to be as healthy and independent as possible. However, members made their own decisions about how this looked and how independent and integrated they wanted to be. Various supports might have included, but not limited to, home care, medication, personal health supplies, nutrition, hygiene, housing, employment, shopping, transportation, community engagement, and chronic and long-term disability management.In addition to planning and working with members, we were responsible for monitoring and empowering the providers of supports that were necessary for each member, to assist our members in achieving their goals.
