  Manage the Department of Perinatal Child Health Care Coordination- focusing on delivering high quality prenatal and postpartum care to patients on the West Side of Chicago and Near West Suburbs. Manage a team of 7 RNs and 7 Care Coordinators coordinating and facilitating care. This work includes collaborating with projects to improve care for patients at PCC and in Illinois. Projects include improvement of maternal mortality and morbidity through coordinating long term primary care follow up of high risk patients with a project with the Chicago Collaborative for Maternal Health and the Illinois Perinatal Quality Collaborative. Another collaborative focuses on expanding access to better miscarriage management in collaboration with the University of Chicago. Show less
