      My debut experience as an occupational therapist involved case management of community aged care and orthopaedics, neurology, hand therapy, paediatrics’ and general medical and community care clients.I gained experience in acute inpatient assessment and rehabilitation, providing advice to patients who had recently been discharged and facilitating their adjustment to temporary or permanent changes and challenges in their lifestyle due to changes in health conditions. I had a wonderful opportunity to work with clients, family members, carers, multidisciplinary team and internal and external stakeholders by providing assessment, education, information, standard and funding reports for client care. I developed from a recent graduate to a confident and experienced occupational therapist in this position. 收起

      As a case manager (for Short Term Restorative Care program), my day-to-day role involved providing day-to-day leadership, operational management, supervision, and planning for the disciplinary team to deliver a sustainable, high-quality service that contributes to achieving organisational goals, including ensuring the program is running at capacity. This encompassed working with the team and stakeholders to deliver a collaborative approach to client care, including planning and undertaking initial assessment, goal setting, care planning and subsequent reviews to meet the client’s goals.Working with the team and management, we implemented significant process and quality improvement in documentation, stand up meetings, decreasing discharge timings, case workloads, and outsourcing services as required.In this role, careful consideration was consistently given to ensure the team practised professional and ethical standards. I was also responsible for contributing to developing and reviewing clinical procedures and protocols for the short-term restorative care team. We as a team achieved almost 100% capacity from 15 June 2019 onwards (throughout the COVID-19 phase). 收起 In this role as Occupational Therapist, I provided Occupational Therapy services to a caseload of complex community aged care clients and people with disabilities for people in some regions of the South Gippsland Region. The role involved completing an assessment, planning, and implementing appropriate intervention strategies that meet client needs whilst promoting functional independence and ensuring environmental safety.The role involved training and prescription assistive technology equipment to address and assist client needs. I regularly liaised with key internal and external stakeholders regarding the care of clients, goal setting, and putting together appropriate care plans. The role also involved conducting a wide range of health promotion strategies for individuals and groups. The role assisted me in enhancing my knowledge of relevant community resources, information, and referral processes and utilising this knowledge in my clinical practice.I regularly completed minor and major home modifications, including assessments and recommendations to increase safety and independence. I also conducted specialised assessments, including seating, wheelchair, scooter, bed, mattress, pressure care, hoist, manual handling, car modification and general day aids.
