  Honored to lead interdisciplinary home and facility hospice team located in Oakland, CA. Steer end of life discussions within the IDG to best serve patients and families on our service. Address increasing acuity levels of hospice admits; hire, train, and educate staff on palliative care directive. Develop community partnerships with ALF/SNF/LTC facilities to layer care partnerships. As a nurse case manager with Oakland's home hospice team I previously provided direct end of life patient and family care in a challenging community with the support of our excellent interdisciplinary team.
