Foci has developed an entirely new approach to care utilizing the best elements of the disciplines of palliative care, concierge medicine and home care in a new payment model that incorporates direct payment by payors in a subscription model. This allows for a shift from the dominant paradigm of fee for service, which has had limited success in both caring for complex patients as well as limited sustainability and profitability. Our model is more efficient than the traditional approach which allows our clinical team to spend more time with complex patients and their families to ensure that patients get the attention and care they need and want. Partnering and shared decision-making as well as a focus on vital conversations are utilized to place the patient at the center of their own care.
