  As a Provider Performance Liaison (PPL), I develop quality partnerships with providers to close identified gaps in care and achieve targeted thresholds in Alternative Payment Model (APM) programs and customized plans. I work closely with providers, community-based organizations, and internal partners to implement interventions that impact improved quality outcomes for CareSource lives. Perform prospective, retrospective, and concurrent review of inpatient and outpatient admissions/procedures. Coordinate care and facilitate discharge.
