  Pioneered new position at rural Hospice agency assisting bereaved families with grief needs. Social worker completed initial bereavement assessment and formulated related treatment plans. Social worker offered supportive counseling, home visits, and phone calls for support. Social worker facilitated support groups, memorial services, and day-long grief events. The voluntary participation by members increased the need for rapport building and relationship maintenance.   The lead social worker is responsible for clinical actions from intake and assessment to discharge. The lead social worker provides oversight to full time and PRN social work team. The lead social work position encompasses intake and is responsible for completing clinical assessments for potential patients and communicating collaboratively with multidisciplinary team regarding admissions. The lead social worker provides a full range of social work services within commonly accepted standards of social work practice which include; individual, family and group counseling, discharge planning, case management and other services as appropriate. The social worker creates and explains treatment plans to patients and their families, maintains patient records, and monitors progress toward treatment goals. Individual, group, family, and progress notes are documented in Meditech.
