      Working within the SHS sector with Men experiencing Homelessness. Specialising in Intake and Assessment.- Within this role I coordinate across all Uniting Youth, Women and Families services covering Western Sydney districts to allocate crisis and transitional accommodation support and early intervention services for people experiencing or at risk of homelessness. - My approach to Intake and Assessment is trauma informed, and as Intake Officer I regularly review and update assessment processes to ensure information gathering is non-invasive and a safe experience for the consumer.- Conduct risk assessments for consumers, developing action plans to mitigate risks, follow up support and provide referrals.- Maintain consultation and relationships and work collaboratively with other services providers, internal case managers and management.- Maintain data, reporting management and consumer information.
