  Connections 2 Success is an organization built to welcome successful strategies and assist in the implementation of positive outcomes with our Senior population or at any age that is experiencing a decline in their activities of daily living baseline. With the promotion and implementation of successful strategies, individuals will rebuild or restructure the ability to care for themselves; promoting self confidence, achieve realistic goals and remain independent. Connections 2 Success also provides minimum oversight of the graduate in order to maintain and monitor the individuals success with independent tasks, achievements and goals. If needed, C2S will act as an advocate in addressing personal health, medical concerns, activities that connect one to community and church, or a representative with insurance, family, or current lifestyle decisions or concerns. Connections to Success is not only a support system for the individual, but a direct connection with the family through verbal or written reports based on the needs of the individual or concerns of the family members.
