  Clinical Liaison

  Acting as liaison between our hospital and the patients, physicians, nurses, case managers, and administrators of our referring hospitals. Manage incoming referrals, do clinical assessments, meet with patients and their representatives to help them make informed decisions about their continuing healthcare needs. Also responsible for the position of Lead Clinical Liaison for our Denver hospitals which includes training, outreach, team building, and conflict resolution.

  Maintain Me is locally owned. We focus our talents and efforts on supporting older adults. Our collaborative team consists of experienced licensed, and non-licensed professionals whose primary role is helping the older people in our community remain as independent as possible while making their own decisions regarding their life goals and living situations. Our transition services are paid on a referral basis from the Independent Living, Assisted Living, or Memory Care our clients chose to transition to. We want to help our community members "maintain" what makes them who they are! We work to remove the frustration and confusion one can feel when looking for services, while imparting decision making powers in what can feel like a powerless situation. For the older adults who are not ready to move to a community, but need more help to remain safe and independent, we offer case management services for an affordable rate. Maintain Me also partners with other community organizations such as financial management, organization and moving, and real estate professionals who specialize in working with our older adult population.   Open Arms Assisted Living is open, and fully operational. We deliver person-centered care in a brand new facility, with state of the art technology that provides telehealth capability in each private room. We wserve older adults in Montbello, and the surrounding areas that are low-income or on medicaid. We also offer inter-generational gathering spaces including the attached church and sanctuary of Saint Andrew Lutheran Church.
