Translated documents from English to Spanish. Posted monthly submission of Medicaid, Medicare and HMO capitations. Delivered bank deposits on a weekly basis. Distributed incoming and outgoing mail internally and to post office.

      University Hospital launched its Population Health Program in February 2019. As part of this launch I began my role as a Population Health Navigator and six months later, was promoted to Community Healthcare - Population Health Manager. Our primary focus is to improve the health outcomes of Newark Residents by monitoring and identifying individual patients that are high utilizers of the Emergency Department; usually based on medical conditions and social determinants. While managing a team of Community Healthcare Chaplains, Community Health Workers and a Patient Navigator, coaching and providing them with guidance to be successful in their roles providing patient care management. Also aggregating data to providing a complete clinical picture of each individual patient to easily identify the results of care while reducing re-admission rates. As a Population Health Manager it is critical to identify and address care gaps within the patient population, especially in communities with chronic illnesses and diseases. It's important to understand outcomes from data that is collected from the Community Healthcare Chaplains and Community Health Workers. The collection and analysis of the data, helps make recommendations based on what the evidence shows. Keeping a close relationship with outside Community Organizations is also key to my role. As we learn patients' social determinants and making connections with Community Organizations helps patients with their health care and social needs. Show less As a Patient Navigator for the Center for Advanced Liver Disease and Transplantation I assisted my caseload of Patients through the healthcare system. This includes assisting the patients through the pre-transplant screening, diagnosis, treatment, and follow-up of a medical condition of liver disease and end stage liver failure. As their patient navigator I assisted patients communicate with their healthcare providers and understand the information they need to make decisions about their health care. Coordinated appointments for future medical appointments, radiology/cardiology testing, medical specialty procedures, financial, legal, and social support. Also worked diligently with referring physicians, insurance companies, employers, case managers, lawyers, and others who may have an effect on the patient’s healthcare needs. As the Senior Patient Navigator, training of a growing team of Patient Navigators was also my responsibility. Our role was also known as patient advocate, patient liaison in the healthcare community. Show less As a Patient Navigator for the Centers for Medicare/Medicaid Services our main focus was to reduce disparities in Cancer Care for the Latino Medicare Population. In doing so I worked closely with the Hispanic elderly population in the Newark Metro area and surrounding counties providing outreach, health education and facilitation of much needed services for patients and their families. My efforts resulted in reducing barriers while ensuring patients received the best available screening, diagnostic and treatment options. My role was key in bringing medical professionals, hospital resources and patient families together to develop a multidisciplinary team approach to their health care. Acted as a patient advocate/liaison to begin engagement efforts for patients with Community Based Organizations and State agencies.
