  Transition Coordinator TCLI (Transition to Community Living Initiative)

  Drawing on my expertise within the human services field, I support individuals transitioning from psychiatric facilities and adult care homes into permanent housing within the community. Throughout client stays, I along with other staff work to provide clients with the tools and independent living skills needed to be successful upon transitioning to permanent housing. Ongoing, I engage with clients for the purpose of creating individualized treatment, personal care, and crisis plans, ensuring that all are specific to their unique needs. Within this role, I work ongoing to foster relationships with social services, government agencies, homeless shelters, and human service providers, to assist clients in gaining access to housing options and medical related treatment/services.
