      • Supervised 16 x 1 bedroom flats let to 16-25 year olds with a history of rough sleeping. Managed rental income to ensure all residents had a care plan, handled evictions and tenancy breaches and organised meetings with statutory / voluntary organisations. Actions ensured compliance with company and legal requirements.• Implemented team management that carried out key worker support and design of individual support plans. This enabled young people to maintain own tenancies and manage necessities such as budgeting, bills, benefits and shopping, in addition to addressing health, accessing employment and education or training as appropriate.• Liaised with social services and health care professionals to ensure support plans were relevant and continuous. This ensured their physical and mental wellbeing were addressed in terms of access to medication and methadone and make certain income was maintained.• Prepared staff rota’s and provided on call cover.
