We are part of the Pediatric Research in the Office Setting Program through the American Academy of Pediatrics. We have our Level III Medical Home Certification from NCQA. The care coordination happens through the partnership among DSS, CCNC, and the pediatric care team. Each partner will have the essential health information of each child when they enter into foster care, which allows for the team to make informed decisions about the child’s care. The child is then is followed by a medical home and see’s their doctor at an advanced rate, so that any health, developmental, socio-emotional, or dental concerns are appropriately addressed.
