      My current role is that of a Programme Coordinator within the Together As One Project, where I have excelled in building positive relationships with key stakeholders, such as Local Authorities, volunteers, support workers, organisations, and participants (18+), in order to bring people in the community together through shared interests. Making a positive change to health and social care. Helping individuals to build friendships, learn new skills, and to help improve their wellbeing. All whilst being safe in the community with their social support. Key objectives of my role are:• Provide coaching and ongoing support of the new database to key stakeholders and customers.• Build and maintain positive relationships with organisations, local authorities, healthcare providers, and participants.• Organise and deliver activities to members of the community, via face to face and virtual meetings.• Producing risk assessments for all programmes delivered. • Monitoring programmes and dealing with any escalations.• Presenting introductory and informative meetings with organisations to sign local authorities and provide ongoing support.• To ensure that the group sessions on the database are customer focused I hold focus groups and meetings to obtain customer feedback and build understanding of the database.• Being the local subject matter expert in the new Together As One project.• Produce marketing and social media content for the project.• Support the development of the webpage. • Deliver webtool training to customers and staff.• Testing of the new database to provide feedback and identify improvements with the customer in mind.• Deputise for senior management in key stakeholder meetings and providing training support to members of the team.• Creating a resource of accessible activities researched throughout Wales.• Produce monthly reports of outcomes and targets met. Show less Within my role as Programme Coordinator for the Can Do Project, I recruited new participants aged between 16-35, and volunteers to take part in the Can Do Programme; liaising with colleges, schools, daycentres and other organisations to be involved in the Can Do Programme. Planning and delivering projects for each group, catered to their needs, where they can learn new skills and help their community; helping young people with disabilities to be more engaged in their community. Each project works towards the participants achieving their City and Guilds certificate. I would write a report at the end of a project, showing figures of targets and outcomes met. This would be gathered from information that I would put into the Can Do Database. I would also work closely within the Wales team on regional events, and the wider national events. Ensuring that I adapt to the changes that the charity requires in order to continue making a difference to people’s lives and deliver the charities vision. Show less
