Show less Since 2014, the Prevention Research Center for Healthy Neighborhoods at CWRU has been apart of a CDC-funded collaborative public health initiative developed by the Health Improvement Partnership of Cuyahoga County (HIP-Cuyahoga). The overarching goals are to improve access and opportunity for healthy eating and active living, chronic disease self-management, and clinic to community linkages. Six neighborhoods in Cleveland and the City of East Cleveland were identified as predominantly African American with considerable health disparities linked to poor nutrition, physical inactivity, and disjointed links to quality health services. As Community Health Engagement Coordinator, -Oversee and manage coordination of resident teams that are engaged in Healthy Eating and Active Living (HEAL) strategies-Provide technical assistance to HEAL partners around grant deliverables and intuitional/community goals-Facilitate communication between residents, stakeholders, community leaders and organization partners as well as lead discussions and activities to build collaboration-Tailor and integrate culturally relevant communication and marketing practices for HEAL strategies-Develop community engagement framework and monitor and assess implementation-Engage community sites and stakeholders to implement healthy eating and active living strategies-Generate comprehensive briefs, project summaries and success stories for reporting and evaluation Show less
