Also, I was involved in the management of volunteers for foster families. This involved the administrative work of updating the volunteer database, to recruiting for volunteers.

  With the assistance of my supervisor, I provided case management services to the community living within the key areas of Jalan Besar, Kampong Glam, as well as up to an extended boundary of Mountbatten and Geylang. Additionally, our team conducted a needs assessment event, to better adapt our services to tailor to the needs, challenges, and resources of the community.

  Our team conducted interviews to assess the needs of the Singapore community during the challenging circuit breaker period when the pandemic broke out in Singapore. At the end of our project, we collated our findings into a report and compiled the stories into an E-Book.

  Youth Guidance Outreach Services

  I conducted weekly counselling sessions with the residents and monthly sessions with the residents’ family members at the Community Rehabilitation Centre (CRC). Additionally, I have formulated assessments and intervention plans with other case managers, while ensuring proper documentation. I was also involved in the co-facilitation of multiple group work sessions. We conducted activities, provided psychoeducation and facilitated discussions. Furthermore, I assisted in the development of the Family Intervention manual. Lastly, I assisted in the data compilation and analysis of the Adverse Childhood Experiences (ACEs) among the residents.   Melrose Home is a residential home for children and youths aged between 6 to 21 years old, who are facing child protection issues.I am part of the Social Care Team, offering case management services to the residents. This includes developing and implementing individualised case plans that are aligned with trauma-informed care principles. Through collaborating with external stakeholders, my team is dedicated to guiding our children and youths on their path to healing, with the goal of reuniting them with their families or reintegrating them into their community.
