      Management of the Ohio Hub National On Call Team which consist of 25 APCs and 5 RNs. Responsibilities include setting team direction, resolving problems and providing guidance to members of the team. I adapt departmental plans and priorities to address business and operational challenges while working within a set budget. Provide input to forecasting and planning activities. Collaborate with applicable stakeholders to help define their communications needs and preferences (e.g., business objectives or issues to be addressed, desired outcomes, root cause analysis). Define/implement overall communication strategies based on identified needs of the business. As the CSM of the Ohio Hub I am responsible for the oversight and management of all activities with the national on call team as it relates to the day –to-day operations with afterhours coverage for Ohio, Indiana, Illinois, Wisconsin, Missouri, and Minnesota. Show less Responsibilities include management of acute and chronic illness of residents in long-term care facilities, rehab, ALF, and in the community. The goal of Optum is to ensure the right treatment, at the right time, and in the right place while providing excellent medical care and improving the residents’ quality of life. This provides the ability to keep residents’ in their home environment while accurately diagnosing, treating, and hopefully maintaining their current level of function. A large part of this role includes excellent communication with family, PCP, and nursing home staff to meet or exceed residents’ goals and manage care. While with Optum I worked as both field and after-hours APC. The after-hours role is a multi- state role with current state licensing in of Ohio, Indiana, Illinois, and Wisconsin. This includes telephonic management of members as stated above. The primary focus of an afterhours APC is in management of acute conditions to stabilize the member and prevent unnecessary hospitalizations. Show less
