      Caregiver Homes provides a unique, home and community based service model for elders and individuals with disabilities in need of personal care. These individuals may be at risk for falls, frequent ER visits and hospitalizations, or nursing facility placement but desire to remain in their own home or that of a family member or friend. Within this live-in caregiver model, Caregiver Homes provides the caregiver with the necessary training and assistance to ultimately keep care in the home. In addition the caregiver receives a twice-monthly non-taxable stipend for their commitment. This type of service is known in Indiana as Structured Family Caregiving. In Indiana, Caregiver Homes has 8 offices staffed by care teams comprised of RNs and Care Managers. Caregiver Homes utilizes specially designed communication technology to empower caregivers to provide effective, high-quality care at home while staying connected with medical professionals, as needed. The program was launched in 2005.Structured Family Caregiving is a Medicaid Waiver service, which enables caregivers to make the full-time commitment of caring for elders and individuals with disabilities. A qualified caregiver is eligible for financial assistance in the form of a daily stipend and is supported by a care team including a RN and care manager. Caregivers enter daily notes into an electronic Care Management System, which helps ensures high-quality care from the care team. Show less
