      My first medical rotations after graduation from medical school were spread across the Royal Oldham hospital and the Burnley General Hospital. I spent 4 months each in General surgery (colorectal), Obstetrics & gynaecology, Stroke Medicine, respiratory medicine, Urology and General Practice. I was involved in managing acutely unwell patients, assisting in theatres, practising basic surgical skills, managing in-patients’ health by Applying the ABCDE assessment, performing investigations, interpretation of investigations (blood tests, ECG, ABG, spirometer and peak flow meters, Urinalysis, bladder scans) and commencing an appropriate management plan. A main part of my role was working within a team (doctors, nurses and allied healthcare professionals) to aid safe and effective patient discharges and appropriate escalation of the deteriorating patient to a senior colleague. I was involved in carrying out several audits, Quality improvement projects and undertaking the GCP (good clinical practice) course as an introduction to research. Show less

      As my first job into Obstetrics & Gynaecology, I learnt to perform basic c-sections and manage common complaints on maternity triage - abdominal pain, PV bleeding, reduced fetal movements and hypertension. I started gaining experience with performing fetal blood sampling (FBS) and perineal suturing for episiotomies and 2nd degree tears. I attended PROMPT training and learnt how I can play a vital role within the multidisciplinary team in managing obstetric emergencies. I completed a Quality improvement project where we designed a patient information leaflet for patients attending fertility clinic. This was compliant with NICE guidelines and also reduced inappropriate attendance at clinic when conservative measures has not been undertaken. The leaflet listing out simple measures - like diet and exercise changes, relevant medication for pre-conceptual planning and also the need to attend with their partner, results in less waste of a clinic appointment and therefore better action plans for upcoming clinic visits.       I commenced my ST2 O&G training at the Royal Oldham hospital. First gaining experience with Instrumental deliveries, Complex caesarean sections, working towards being a 2nd on-call/Registrar and working within a team in a busy Obstetric/ tertiary Gynae Unit. I proceeded to become an ST3/Registrar at this unit where I was able to work closely with midwives and anaesthetists in managing both obstetric and gynae emergencies. I was in charge of the departmental teaching which took place weekly and would assign various trainees to the teaching dates ensure a good variety of topics relevant to the GP trainees as well as the O&G trainees were covered. I undertook a fetal medicine project in the form of a retrospective audit looking at the association of Low PAPP-A and IUGR and the possibility of using PAPP-A as a predictor for fetal outcome. This was submitted to the fetal medicine Foundation. I also compiled two case reports: one looking at a diagnosis of SLE which presented as pyrexia of unknown origin and the other on Uterine Rupture which occurred antenatally. These were submitted to the International society of Maternal Medicine Conference and the RCOG Congress respectively. I developed and authored the guideline for polyhydramnios for the Trust as we did not have a local one.I was also on the RCOG Committee for North West Trainees as the treasurer and trainee representative for my Trust.       During my time at Wigan, I covered both Obstetrics and gynaecology on call. I was able to safely manage patients on delivery unit and also provide safe emergency gynaecology care. During the night shifts, I worked a single tier rota managing both obstetrics and gynaecology independently. I frequently debriefed patients on the postnatal ward regarding their deliveries - we had a system where debriefs were made for all instrumental deliveries, PPH and any other complex delivery. It was a good way of reducing misunderstanding among patients regarding their births and complaints. I attended a variety of antenatal and gynaecology clinics. I carried our practical skills like insertion of pessaries for prolapse and vulval biopsies. I was working in this Unit during the COVID pandemic and I first learnt to carry out telephone consultations here and to appropriately triage patients and avoid unnecessary follow-up appointments. I was involved in updating the Breech Management guideline and the ECV patient information leaflet. I undertook an Audit in the Management of prolonged rupture of membranes and highlight a huge gap in our practice where many still treated with antibiotics without clinical signs of infection. The change in guidelines was highlighted as a learning point for everyone.As part of my experience here, I regularly led themed case discussions (TCDS) for medical students who were undertaking their placements in women's health/ O&G.       I commenced my ST5 training in this unit. As a busy unit for gynaecological emergencies, I was able to assist with open procedures (e.g total abdominal hysterectomy) and also laparoscopjc procedures (sterilisation, laparoscopic salpingectomy for ectopic and laparoscopic cystecomies).I learnt prioritisation in a different way as I had to cover the acute gynaecology overnight as the senior reg and would have to prioritise between a gynae emergency and an obstetric emergency. This meant I got used to communicating with the consultant at home early enough to alert them to when they needed to come in and using an SBAR handover to inform them of crucial patient information.I undertook an audit on stillbirth and the management in our unit. highlighting the importance of the Saving babies lives bundle (v2) and its implementation innreducing our still birth rate in the region.I organised the regional teaching for ST3-5's on sexual health. I first undertook a surgery via the trainee group to ensure that topics they wanted were represented. I also mapped each topic to the curriculum to help trainees with completing their Competencies. I contacted a wide range of speakers and we had a succesful online teaching day (as we were stilk in the pandemic). The feedback from the course was very good and I learnt a lot personally about planning an event on a large scale, time management, delegation of tasks and team collaboration.Towards the end of my time at Bolton, I was stepped up to the senior registrar rota and did my on calls at night with a junior reg (ST3). It was a steep learning curve but I was able to navigate the balance between supervising, providing support and advise to my junior colleague. it also taught me a lot with regards to lisiaisng with midwives and handling conflict between myself and the coordinators in a professional manner. I gained confidence in leading ward rounds and making suitable plans for the women on delivery unit. Show less

It is an online platform for summarising key information from guidelines and practice documents, designed to be assimilated in 5 minutes or less. Currently it is operating as an Instagram page but there are plans to make this into an audio resource for trainees in the near future. It hopes to be the 'Spotify' for O&G knowledge so trainees can listen on the go and study for their exams and clinical practice!

      I commenced my advanced training here, undertaking ATSMs (Advanced Training Specialist Module) in Obstetric Medicine and advanced labour ward practice.I've been involved in maternal medicine MDT meetings, clinics, ward rounds and study days. In addition to the dedicated maternal medicine clinic, I have also frequently worked in our Haematology, diabetes, cardiology & HIV clinics.I have built my intermediate and advanced scan skills comprising Fetal biometry, placental localisation, amniotic fluid measurement, umbilical and uterine artery doppler by frequenting the Placenta and Rainbow clinics . I have undertaken quality improvement projects relating to the Specialist obstetric clinics. I have been involved in updating our sepsis guideline and undertaking an audit of our current ECV service.I am currently working on a project that involves leading change in one of our clinical areas within the department. As a senior trainee, I've been working closely with the Medical school at Manchester University by being an examiner for undergraduate examinations (OSCEs/CCAs).I am also involved with the UK academic foundation programme apppications and interviews run by Health Education England. As part of the NW O&G committee, I've helped run practice OSCE's for the MRCOG part 3 exams for trainees. I organised the STEP Up course for the ST2s about to become registrars in the month leading to the changeover covering practical emergency stations and an interactive session on the management of delivery unit.The practical and theoretical aspects of the course were well received and appreciated by the trainees as reflected in the positive feedback I received.
