During our meetings I am overseeing their progress and transition to San Jose State University. I connect my students to resources on campus as well as promote their academic and social growth.

      I was a case manager for the intensive outpatient program (IOP) at Community Solutions. As a case manager I would work with youth as young as six to the age of twenty. The youth would be in intensive care due to the severity of their mental health diagnosis. I would meet with the client's in-person in the community, home, and office setting. I would complete assessments such as CANS 5+, PSC-35, and the county mental health assessment. Additionally, I would collaborate with the youth and their guardians to create a holistic and tailored intervention plan. My caseload would consist of 15 to 20 clients that I met with on a weekly basis to provide rehabilitation services. Rehabilitation services consists of educating and training clients on how to utilize coping skills. I did case management with client's and their caregivers to make sure all their needs are met and that they are connected to community resources. Additionally, I provided collateral services to parents/caregivers to ensure that they knew about their child's diagnosis and taught them ways in which they can support their children. In this role I would also assist with the intake process. This means that I would call the youth or parent to meet and review initial paperwork, gather baseline information and symptoms, as well as complete the initial treatment plan for services. I worked with students from 6 to 18 years old who were having behavioral and mental health challenges. I had a case load of 5 clients that I meet with on a weekly basis to provide rehabilitation services which consisted of educating and training clients on how to utilize coping skills. I also did case management with client's and their caregivers to make sure all their needs are met. Additionally, I provided collateral services to parents and caregivers to ensure that they know about their child's diagnosis and teach them ways in which they can support their children. Lastly, I created and co-facilitated a "Gratitude Group" which focuses on a coping skills and SkillsStreaming curriculum with many kid friendly art and gratitude activities.
